

Two police facilities are available to assist citizens who may need our services. Our headquarters building is located at 650 N. Apollo Blvd., and the Communications and Criminal Investigations Divisions (CCID) building is located at 701 S. Babcock St.

NON-EMERGENCY INCIDENT REPORTING
(If you have an emergency or an incident requiring immediate medical, fire, or police response, DIAL 911.) Citizens with non-emergency situations have a method for submitting reports without first waiting to meet with an officer. Choose between two methods for providing a report:

If you would like to use the online form
that allows you to fill-in the blanks, use the ONLINE REPORT FORM. Once you have filled-in the form, you simply click on
the 'submit' button at the bottom of the page to send it to the
Police Department.

to learn whether your incident meets the criteria for using the online form.CHOICE 2:
Also available is a report form that can be downloaded and printed out. The form can then be either mailed or faxed to the Melbourne Police Department. If you have "Adobe Reader" on your computer, click here to access the form If not, you will first need to download and install the free "Adobe Reader" software in order to view or print the form. Use this link to reach the page where "Adobe Reader" can be downloaded.Mail the completed form to the Melbourne Police Department at 650 N. Apollo Blvd., Melbourne 32935, or submit it by fax to 321/242-0795. The submitted report will be reviewed on the following business day, then entered into the records system. You will be contacted to verify your information and/or to provide additional information if necessary.
